  • I am currently unable to accept patients on Medicare.

  • WHAT TO DO BEFORE YOUR FIRST VISIT:

    • Make sure to request an actual appointment slot before requesting a waitlist spot.

    • After your appointment is accepted, you will receive an email invite from Charm to create a profile for the new patient portal. If you do not get this, please message or call me (you can send a message to the contact us section of this website).

    • Once you create your patient portal, go to “Questionnaires,” you will have access to the assigned patient questionnaires (they are different from the pre-screener form).

    • Remember to complete the questionnaires (available after signing into the patient portal) for new patient consultations before the visit. If the questionnaires are not filled out at least 5 minutes before your scheduled visit, your appointment will be canceled and need to be rescheduled.

    • PATIENTS REQUESTING EVALUATION FOR POSSIBLE ADHD: PLEASE OBTAIN NEUROPSYCHOLOGICAL TESTING PRIOR TO OUR APPOINTMENT. If you have prior documentation, you can fax it to 312-872-7660.
