WHAT TO DO BEFORE YOUR FIRST VISIT:
Make sure to request an actual appointment slot before requesting a waitlist spot.
After your appointment is accepted, you will receive an email invite from Charm to create a profile for the new patient portal. If you do not get this, please message or call me (you can send a message to the contact us section of this website).
Once you create your patient portal, go to “Questionnaires,” you will have access to the assigned patient questionnaires (they are different from the pre-screener form).
Remember to complete the questionnaires (available after signing into the patient portal) for new patient consultations before the visit. If the questionnaires are not filled out at least 5 minutes before your scheduled visit, your appointment will be canceled and need to be rescheduled.
PATIENTS REQUESTING EVALUATION FOR POSSIBLE ADHD: PLEASE OBTAIN NEUROPSYCHOLOGICAL TESTING PRIOR TO OUR APPOINTMENT. If you have prior documentation, you can fax it to 312-872-7660.
We require a credit card on file for your visit, but it will not be charged until your visit.
